Forms
Overview
Section titled “Overview”The Forms page is a gallery of your organisation’s active forms, ready to fill out: intake forms, assessments, checklists and the like. The page heading reads Forms. Anyone in the organisation can fill one in. When you submit, the answers are saved as a submission for that form, which administrators can see in the form’s submissions list in the Form Library.
Common tasks
Section titled “Common tasks”Find a form
Section titled “Find a form”- Click Forms in the sidebar.
- Type into Search forms… to filter by form name or description.
- Each card shows the form’s name, a short description, its category badge, a Required badge if the form is mandatory, and how many fields it has (for example “8 fields”).
- Click Fill out on the form you need.
Fill out and submit a form
Section titled “Fill out and submit a form”- Open a form with Fill out. It is laid out in sections, each with its own heading.
- Work through the fields. Required fields are marked with a red asterisk (*).
- Some fields arrive pre-filled from the client’s record:
- A field marked Already on file, please check is important. Read it and correct it if it has changed.
- A field marked Already on file is informational. Change it only if it is wrong.
- Signature fields are never pre-filled. Type into Type your signature.
- Click Submit when you are done. On success you see Form submitted. and return to the Forms gallery.
- To leave without sending, click Cancel. If you have typed anything, Cura asks Discard this form? first. Choose Cancel in that box to keep working.
Fix a form that will not submit
Section titled “Fix a form that will not submit”- Click Submit. If answers are missing, Cura jumps to the first one and puts your cursor in it.
- A red summary at the top reads, for example, “3 fields still need an answer”. It is grouped by section, and each entry is a link. Click one to go straight to that field.
- Each empty field says what is wanted, such as “Enter First Name.”, “Choose an option for Gender.” or “Enter a date for Date of Birth.”
- A wrong format reads differently. An email without the right shape says “Enter a valid email address.”
- Fix the fields and click Submit again. The count goes down as you fix them.
Save a draft and finish later
Section titled “Save a draft and finish later”- Partway through a form, click Save draft. You will see Draft saved.
- Leave the page. When you open the same form again, your answers are still there.
- Finish the form and click Submit. The draft is cleared.
- Only active forms appear in the gallery. Forms are made and activated by an administrator under Settings, then Form Library.
- You keep one draft per form. Each Save draft replaces the last one. Drafts are yours alone and do not show in the submissions list.
- Error messages update when you click Submit, not as you move between fields.
- Forms that go to a client or participant by link are different. Those answers wait in Forms to Review (see that help page).
- Use the search box rather than scrolling. The gallery shows every active form.
Troubleshooting
Section titled “Troubleshooting”- “No active forms yet” / “An admin needs to create and activate a form before it appears here.” No forms have been activated. Ask an administrator to activate one under Settings, then Form Library.
- “No forms match your search” / “Try adjusting your search terms.” Clear the search box and look again. The form may be named differently than you expect.
- “N fields still need an answer” Use the links in the red summary to find each field, answer it, and click Submit again.
- “Network error. Please try again.” The submission did not reach the server. Check your connection and submit again. Your answers are still on screen.
- I submitted a form but can’t find it in Forms to Review. Forms filled in by staff here do not go to Forms to Review. Look in the form’s submissions list instead.
